BlephEx treatment: deep eyelid margin cleaning

Eyelid margin inflammation can be improved by regular eyelid margin cleaning. This also improves dry eye syndrome. The aim is to remove deposits on the eyelid margin and reactivate any blocked sebaceous glands. These stubborn crusts on the eyelid margin are often difficult for patients to remove completely at home using conventional mechanical eyelid margin cleaning. Therefore, an instrument for mechanical abrasive cleaning of the eyelid margin can be a valuable aid in therapy.
BlephEx therapy involves deep cleaning of the eyelid margin using a BlephEx device. The BLEPHEX device works like a small drill with a rotating, replaceable head that gently but thoroughly cleans the eyelid margin with an additional cleaning solution if necessary.

This cleaning is carried out at our practice and takes approximately 5 minutes. In cases of severe inflammation of the eyelid margins, this treatment should be carried out twice a year.

How does BlephEx therapy work?

The cleaning process involves two steps, similar to tartar removal at the dentist:
Step 1: Performed at home. You clean the edge of the eyelid regularly; we will show you how.
Step 2: Performed by us. The edge of the eyelid is professionally cleaned every six months.

IPL treatment with E-Eye

Intense pulsed light (IPL) – intense light therapy for Meibomian gland dysfunction (MDD)
IPL is a light therapy that uses very intense light – visible white light from a flash lamp. The flashes of light are directed at the skin around the eye. Due to the very high light intensity, the eye itself must be covered.

IPL comes from the field of dermatology and was developed from the original use of laser light. Unlike laser light, which only contains light of a specific wavelength, IPL uses light across the entire visible light spectrum. The IPL light therefore appears white.

Harmful light frequencies, especially ultraviolet (UV) light in the broad blue range, and sometimes infrared (IR) heat radiation, are filtered out.
The light has a very high intensity.

According to various studies, IPL can improve meibomian gland dysfunction (MGD), even in cases that previously appeared resistant to treatment, especially when the meibomian oil gland disorder occurs in conjunction with skin diseases such as rosacea (acne rosacea, facial redness).

IPL is performed in therapy cycles of 3-4 sessions at intervals of several weeks. A typical therapy regimen consists, for example, of two sessions at intervals of approximately 14 days and a further two or more sessions at intervals of approximately one month or until an effect is achieved.

During a session, 4-5 flashes of light are usually applied to the skin around the respective eye. Due to the strong light intensity, the eyes must be closed and the eyelids additionally covered with a dense protective cover; the doctor must also wear protective goggles. This ensures that the eyelids themselves and the Meibomian glands inside them are not exposed to direct light radiation.

The mechanism of action of IPL treatment for meibomian gland dysfunction (MGD) is not yet fully understood, even though several studies have shown positive results. One possible explanation is the potential absorption of IPL light by the natural iron pigment in red blood cells. This can lead to a corresponding warming of the blood, followed by an increase in skin and gland activity. The destruction of red blood cells and thus the destruction of proliferated vessels is another hypothesis. This is important in chronic eyelid inflammation (blepharitis) and in any associated rosacea (skin redness) caused by dilation of the blood vessels.

Several other potential positive effects are suggested. These range from a possible antibacterial effect to possible anti-inflammatory effects and influences on the nervous system to warming or direct improvement of glandular metabolism, etc.
